Insurance Claim Procedure

1. Complete claim documents

2. Submit all documents to Dai-ichi Life Vietnam

3. Be advised of the insurance claim result

Funeral Benefit

Required documents are:

a. Claim form (Download here).
b. Certified copies of the ID card of the Policy Owner/ Claimant/ Payee
c. Certificate of first-row inheritance and written authorization (if the Policy Owner is the main Life Insured who is dead and without any Beneficiary being appointed), or the Legal Guardian Approval for the Beneficiary (if the Beneficiary is below 15 years old and has no father and mother).
d. Certified true copy or extract of the Death notice of the main Life Insured


Death Benefit

a. Administrative documents:

  • Claim form (Download here)
  • Original Insurance policy
  • Certified written authorization for Dai-ichi Life Vietnam to collect information and medical profiles of the Deceased

b. Documents for insurance benefits substantiation

  • Certified copy of ID card of the Policy Owner/ Claimant/ Payee.
  • Certificate of first-row Inheritance and written authorization (if the Policy Owner is the main Life Insured who is dead and without any Beneficiary being appointed).
  • Legal Guardian Approval for the Beneficiary (if the Beneficiary is below 15 years old and has no father and mother)

c. Documents for death substantiation

i. Death by illness or accident:

  • Certified extract or copy of Death Certificate
  • Copy of all medial treatment files
  • Copy of healthcare insurance card (if any)

ii. Claim for Death by accident:

  • Certified extract or copy of Death Certificate
  • Accident profile provided by investigating Police (certified by the investigating Police Authority), comprising Crash diagram, Crime scene investigation, Autopsy minute, Accident settlement minute, Forensic minute.
  • Medical profile in summary for accident emergency, laboratory tests (CT scan, X-ray, MRI... in case of any accident emergency at the hospital).

Coverage for Hospital Stay due to Accidents Benefit

Required documents are:

i. Claim form (Download here)
ii. Certified written authorization for Dai-ichi Life Vietnam to collect information and medical profiles of the victim.
iii. Copy of the ID card of the Policy Owner/Claimant /Payee.
iv. Certified original or copy of the Hospital discharge paper.
v. Copy of the detailed billing documents for inpatient treatment and the Receipt of hospital charges.

Disability by Accidents Benefit

Required documents are:

i. Claim form (Download here)
ii. Certified written authorization for Dai-ichi Life Vietnam to collect information and medical prifles of the Policy Owner/ Life Insured
iii. Copy of the ID card of the Life Insured/ Claimant/ Payee.
iv. Certified original or copy of the Emergency medicine prescription/ Disability document.
v. X-ray, CT scan, MRI and ultrasound films and results.

Critical Illness Benefit

Required documents are:

i. Claim form (Download here)
ii. Original Insurance policy.
iii. Certified written authorization for Dai-ichi Life Vietnam to collect information and medical profiles of the Policy Owner/ the Life Insured.
iv. Copy of the ID card of the Life Insured/ Claimant/ Payee.
v. Biopsy results.
vi. Original X-ray, CT scan, MRI, ultrasound films and results.
vii. Certified original or copy of hospital discharge paper.
viii. Certified original or copy of the medical profiles in summary. 


Total and Permanent Disability due to Accident Benefit

Required documents are:

i. Claim form (Download here)
ii. Original Insurance policy.
iii. Certified written authorization for Dai-ichi Life Vietnam to collect information and medical profiles of the person suffering from the illness or accident.
iv. Copy of the ID card and household registration book of the Claimant/Policy Owner/ Payee.
v. Certified original or copy of the hospital discharge paper and Medical profiles in summary.
vi. Original X-ray, CT scan, MRI and ultrasound films and results.
vii. Original documents related to medical treatment of illness/accident.
viii. Minute on the percentage of work capability loss provided by Board of medical examiners (conducted after 6 months from the date of arising risk).
